The course (medicine block) is divided into 3 blocks rotated among students, each block is 5 weeks and includes one third of the batch both male and female students simultaneously. A weekly announced themes (3-4 themes) that is related to specific systems, e.g., cardiorespiratory, endocrinology, dermatology, gastroenterology, infectious diseases. It also includes several clinical situations which will be opened to all students as online learning content that is defined by a week ILOs and a week syllabus (topic list).
Students are expected to go through the online content on their own and approach staff responsible for the week if they encounter any difficulty. Students learning activities is managed through two types of activities: centrally defined teaching and learning activities and clinical placements activities. Evidence of acquisition of skills should be collected by the student in a logbook.
A- Centrally Defined Teaching and Learning Activities
These will be delivered through a 2 days of college-based activities every week, during which several structured sessions with predetermined content are provided.
1. Four-hours CBCL sessions combined with students-led guided objective tutorial activities that link to the online content and weekly theme; during which the students in a team-based learning environment will be guided through a clinical reasoning discussion of new cases by an expert.
2. One-hour PCC session during which variable topics which are related to patient-centered clinical practice. This session will be delivered either as a large interactive workshop or small groups activities.
3. Lectures (new topic and applied basic topics): 1-2 hours per week
4. Clinical debrief for 2 hours weekly and led by an experienced clinician. During this session students will bring:
? The experiences gained from the field and discussing the clinical experience of each student separately during his weekly hospital stay among themselves and with their tutor.
? Case management discussion from 2 students according to case management list informed to the students. The main objective of these case management is to encourage the students to look at the scientific background of at least 2 different topics in the medicine. They will revise the basic and clinical background of these subjects aiming to improve the clinical reasoning abilities and to recognize and complete knowledge gaps. The discussion will start with the full presentation of the case history and clinical examination, then the reasoning for the provisional diagnosis, differential diagnosis and the investigations to reach the definitive diagnosis and finally the management options for the patient. Moreover, the student is expected to explain the pathophysiological mechanisms involved in the patient symptomatology.
5. Two -hours prescribing session during which two prescribing exercises that match the weekly CBCL cases will be delivered, e.g., oxygen therapy, inhalers, or antihypertensives medications.
B- Clinical Placements
? Students will rotate through three clinical placements (one in medical environment in the distributed main hospital for 3 weeks and one in infectious diseases hospital for one week , and one in different dermatology clinic of different hospitals for one week ). During clinical placements, students will be given the opportunity to be part of the clinical team shadowing allied health care staff, nursing staff and junior doctors learning new skills and applying the previous learnt skills. A predetermined minimum standards are required to pass the placement successfully that will include predetermined attendance and activities in the field.
? During the course and mainly during the time spent in the field, students are expected to be able to independently take histories for various common patients’ complaints. In addition, students will be expected to independently perform all major systems examinations on real patients. Teaching of these systems will extend the previous learning in Years 2 and 3 through the consultation skills activities during the Early Clinical Experience (ECE) sessions; and completed in year 4 and 5 through hospital bedside teaching during the clinical placement.
? Students will have a weekly clinical assessment/evaluation by a faculty member for 2-3 students with teaching feedback for all students.
C- Evidence of clinical skills acquisition (the Logbook)
Pre-determined minimum requirement should be collected in every placement in a student logbook using specific forms. These requirements are distributed between the different skills that student should practice and provide evidence of going through them.
